Team Forms:
- Comp and Select teams may place
their orders for uniforms by submitting a Uniform Request From.
- All Comp and Select teams are
required to track their team expenses and report them in summary form to the
Orchard Valley Board of Directors using the Statement of Receipts and Distributions form.
- The Tryout Release Form should be used whenever a non-registered player
seeks a tryout within Orchard Valley.
- On-line registration for Comp and Select players is
available here. Only players selected through a formal tryout may
register through this process.
- Player passes must include a photo of the player.

This is where you can find the
Class 1 or Class 3 coaching application
You need to fill out the form and deliver it to the league address on the bottom of the form. You must also
possess a USYSA-sanctioned coaching license.
- The Orchard
Valley Youth Soccer League is now accepting resumes from anyone interested in
serving on its Board of Directors. The application
form is here. Please indicate which position(s) you
might be interested in and mail along with your resume to P.O. Box 0251, Morgan
Hill, CA 95038 or FAX it to 779-6696.
